Using Provider Forms for diagnosis queries, physician notes, and Medicare A certifications

Prepare physician diagnosis queries, physician notes, and Medicare A certifications from the resident's record, export them for signature, and track what is outstanding across your facilities.

Provider Forms is a tab on each resident in Admissions. It lets you prepare the three provider documents SNF teams chase most often, using what is already in the chart: a diagnosis query to the physician, a physician note that attests to active conditions and skilled services, and the Medicare A certification and recertifications. Anthuria prefills what it finds in the record, you review and edit, and you export a PDF for a wet signature. Provider Forms is available on select accounts.

See what is outstanding

From the Admissions list header, click Outstanding provider documents. The page lists, across your facilities, every drafted or printed document still awaiting signature and every unsigned certification that is overdue or due within 7 days.

  • Count chips filter the list: Overdue, Due in 7 days, Awaiting signature, and Draft.

  • Filter by facility. Overdue items sort to the top.

  • Click a line to open that resident's Provider Forms tab.

Open the Provider Forms tab

Open a resident from Admissions and click Provider Forms. Choose New query, New physician note, or New certification. Saved, printed, and signed forms are listed under Provider Documents, and Medicare residents have a Certification and Recertification schedule below.

Send a diagnosis query

Use a query when a diagnosis appears in the record but no physician, NP, PA, or CNS document establishes it.

Start the query

  1. On the Conditions tab, click Query provider on the condition, or click New query on Provider Forms and choose the diagnosis.

  2. Facility, resident, and the selected diagnosis are prefilled. Enter the primary physician and confirm the date of query.

  3. To query more than one diagnosis on the same form, use Add another diagnosis. Up to three diagnoses can be included.

Describe the documentation gap

Each diagnosis shows the clinical issue and an editable Documentation gap. Edit it to state what is missing from the record and why the diagnosis cannot be supported as written.

Choose the documents to send

  • Where this code came from: the notes the diagnosis was found in are checked for you.

  • Also on the chart: check any other documents the physician needs in order to answer.

  • At least one attached document must be physician, NP, PA, or CNS authored, or nothing on the query can establish the diagnosis. The form tells you when nothing on the chart is physician-authored.

Add clinical indicators

Check the indicators that apply: diagnostic result supports the condition, medication ordered for this condition, ongoing monitoring documented, therapy evaluation supports the condition, acute exacerbation documented, or physician assessment documented. For each one, pick the document that shows it. An indicator with no document will still send, but the physician will have nothing to review for it.

If the supporting document is not in the list, such as an in-house lab result, enter its title, date, and author and credential, mark whether it is physician-authored, and click Add.

Preview, edit, and export

  1. Add any comments, then click Preview and export. Use Save draft if you are not ready.

  2. On the preview, click any value to change it, for example the clinician reviewing or the medical record number. Labels, checkboxes, and the signature block are fixed. Check Attach medication list on the form if the physician needs current medications.

  3. Click Export PDF and print the query for a wet signature.

The physician marks each diagnosis Confirmed, Not Supported, or Needs Further Evaluation, adds a clarified diagnosis if different, and indicates whether the condition was present on admission. The query states that it is a request for clarification and does not suggest a specific diagnosis.

Prepare a physician note

The physician note documents that the resident's active diagnoses were reviewed and are being treated under the provider's plan of care, and supports the need for a skilled stay.

  1. Click New physician note. Facility, resident, and room are prefilled. Enter the date seen.

  2. Under Active conditions, conditions found in the record are prechecked and tagged on this chart. Sections include Asthma, Pulmonary Disease or COPD; Cerebral Palsy; Multiple Sclerosis; Parkinsons; CVA, Hemiparesis or Hemiplegia; wounds, pressure ulcers, and open lesions; respiratory therapy; and IV fluids or IV medications.

  3. Add the site and treatment plan, medical rationale, other diagnoses, diet consistency, and swallowing issues where they apply.

  4. Check the Skilled services provided, then click Preview and export and Export PDF.

Prepare a Medicare A certification

For Medicare stays, Provider Forms shows a certification schedule with each due date calculated from the date admitted to Medicare A: the Initial Certification at the time of admission or as soon as possible after, the 14-Day Re-Certification on or before day 14, and each 30-Day Re-Certification no later than 30 days from the previous one.

  1. Click a certification on the schedule, or click New certification and choose the certification type.

  2. Resident information is prefilled: facility, resident name, Medicare number, date admitted, and first skilled day. Enter the medical record number and attending physician.

  3. Check the Reason skilled care is required.

  4. Review Skilled services provided. Services found in the record are prechecked and tagged pre-filled, with the evidence listed underneath. Adjust anything that does not match.

  5. Complete estimated duration, post-discharge plan, whether the continued stay is for the same condition, and additional comments. Complete Reason for delayed certification only if the certification is signed after its due date.

  6. Click Preview and export, then Export PDF.

The Primary diagnosis is shown to help you prepare the form. It does not print.

Mark a form signed

Exported forms show as Printed on Provider Documents. After the physician signs:

  1. Click Mark signed next to the form.

  2. Enter who signed it and the date signed.

  3. Click Mark signed. The status changes to Signed and the form leaves the outstanding list.

Questions we get

Does Anthuria send the form to the physician?

No. You export a PDF and route it for a wet signature the way you do today, then mark it signed in Anthuria.

Can I change a prefilled value?

Yes. Prefilled values are a starting point. Edit them on the form, or click a value on the preview to change it before export.

Who can sign a certification?

The physician. An NP, CNS, or PA may sign only if they have no employment relationship with the facility. A SNF may substitute utilization review for the second and subsequent recertifications.

Provider Forms uses AI to prefill information from the medical record. All prefilled content should be reviewed by your clinical team before export, and all determinations are made by the signing physician. Screenshots in this article show masked demo data.
