Using Admissions to review new residents and add ICD-10 codes to PCC (Beta)

Find new admissions, search their hospital documents, review ICD-10 and medication recommendations with the evidence behind each one, and add codes to PCC. Admissions replaces Patient View.

Admissions is where you review every newly admitted resident and their hospital documents. Anthuria pulls the discharge summary, H&P, therapy evals, referral, and insurance documents from PCC, reads every page, and recommends ICD-10 codes and medications. Each recommendation links to the exact passage it came from, so you can verify it in seconds and add it to PCC.

Admissions replaces Patient View. Everything you used in Patient View is here, along with an admissions list, a Medications tab, a Provider Forms tab, and new filters. You will see a Beta label next to Admissions in the menu.

Open Admissions

In the left menu, under Clinical, click Admissions.

The list shows each new resident with their facility, admission date, status, location, payer type, and the number of documents uploaded. Click any column heading to sort.

Find the residents you need

Use the filters above the list. You can combine them.

  • Facility: choose one or more buildings. If you have access to more than one facility, choose your facility before opening a resident.

  • Status: select Admitted (in PCC). Click Clear to remove it.

  • Payer Type: Managed Care, Medicaid, Medicare A, Other, or Private.

  • Admission date: pick a date range on the calendar.

  • Has documents: turn on to show only residents with documents uploaded. Turn off to see everyone.

Click a resident's name to open their record. The record has four tabs: Search, Conditions, Medications, and Provider Forms.

If you see Still processing, documents are being reviewed, Anthuria is still reading the files. Conditions appear when it finishes. Click Back to list and open another resident in the meantime.

Search the hospital documents

The Search tab lists every document for the resident with its category, uploaded date, and saved search tags.

  • Filter by Category or Uploaded date, or type in Search by document name to find a file such as the discharge summary. Categories come from your PCC miscellaneous document categories.

  • The Saved Search Tags column shows which documents mention topics like Diabetes, Fall, IV Fluids & Meds, and Mechanically Altered Diet.

To search inside the documents, type a word or phrase, for example fall, and click Search. The Results panel shows how many hits were found and on which pages. Click a result to jump to that spot. The word is highlighted in the document.

Search finds the exact words you type, like Ctrl+F. It does not interpret clinical meaning, so review each hit in context.

Use Add Search Group to look for several related terms at once, and Clear to start over.

Review ICD-10 recommendations

Open the Conditions tab. The ICD-10 Codes card shows how many findings there are. The table below lists each condition with its ICD-10 code, MDS category, whether it is already in PCC (In EHR), whether it is Resolved, and the source documents.

Recommendations come from documents uploaded in the last 14 days. This keeps old hospital records from a prior stay out of the list for readmitted residents. Click Show all instead to include every document.

Show only codes not yet in PCC

  1. Click Filters.

  2. Under In EHR, check No.

  3. Optionally set Resolved to Yes or No.

  4. Click Apply Filters.

Click any column heading to sort, for example by MDS Category to bring NTA conditions together.

Check the evidence

Click a condition to see the documents and pages it was found on, with the reason that code was chosen. Click the page link to jump to the highlighted text.

Inside the document, click the code chips at the top (for example E78.5 or I10) to move between every diagnosis Anthuria tagged. Switch between Insights View, a searchable version with every code tagged, and Original PDF to see the document exactly as it came from the hospital.

Code specificity follows what the hospital documented. If the note only says "HLD," you will get E78.5, hyperlipidemia, unspecified. If the fracture site is documented, you will get the specific code. For fractures, the hospital's initial encounter code (7th character A) is recommended as the subsequent encounter code (7th character D) for the SNF stay.

If a condition needs physician documentation before it can be coded, click Query provider in the Actions column to start a diagnosis query. See Using Provider Forms for the full steps.

Add ICD-10 codes to PCC

  1. On the Conditions tab, filter to In EHR: No.

  2. Review each recommendation against the source passage.

  3. Click Add in EHR. You are taken to the resident's diagnoses in PCC to enter it.

  4. Give the recommendation a thumbs up or thumbs down. Your feedback improves future recommendations and shows as a column in the export.

Add notes for your team

Open the notes panel on a resident to add a note with a title, text, and tags. Set Visibility to Private or My organization, then click Save. Notes can be downloaded as a CSV from the Export menu.

Review medications

Open the Medications tab to see every medication Anthuria found, with its linked ICD-10 code, status, last order, route, dose, frequency, and where it was documented (for example the discharge summary or treatment plan).

  • Filter by Last Order, Route, Location, Frequency, or Documents. Click Clear all to reset.

  • None found in the ICD-10 Code(s) column means no supporting diagnosis was identified in the documents. This is worth a look before the MDS.

  • Scroll to Documents to see which files each medication came from, and open them in Insights View or Original PDF.

  • Use Add in EHR to go to PCC, or Export to download the list.

Export ICD-10 codes and notes

From the Admissions list, click Export in the top right.

  1. Click ICD-10 codes.

  2. Choose the Facility.

  3. Under Extract from patients admitted within, choose Last 7 days, Last 14 days, Last 30 days, or Custom range.

  4. Under Extract from documents uploaded within, choose Last 14 days or Across all time. We recommend Last 14 days.

  5. Check the resident count at the bottom, then click Download.

Notes

  1. Click Notes.

  2. Choose the Facility, then the Patient Name.

  3. Set Notes that are visible to, Timeframe, and Status.

  4. Click Download CSV.

Outstanding provider documents

The Outstanding provider documents link in the Admissions header opens a list, across your facilities, of every provider document still waiting on a draft, print, or signature. See Using Provider Forms for details.

Questions we get

What happened to Patient View?

Patient View has moved into Admissions. ICD-10 extraction, condition search, Add in EHR, thumbs up and down, bulk export, and Insights View all work the same way. Admissions adds the admissions list, filters, the Medications tab, and the Provider Forms tab.

A resident shows 0 ICD-10 codes

The documents may still be processing, or nothing was uploaded in the last 14 days. Check back shortly, or click Show all instead.

I can't find a resident in the list

Check your filters. Turn off Has documents, clear Status, and widen the Admission date range. The list only shows facilities you have access to.

Does Add in EHR enter the code for me?

No. It takes you to the resident's diagnoses in PCC, where you enter and confirm the code. Nothing is written to PCC without you.

Why is the code unspecified?

The code is only as specific as the hospital documentation. The rationale under each recommendation explains what was documented and why that code was chosen.

The exports have resident information in them. How should I handle them?

Exported files contain protected health information. Store, print, and share them according to your facility's privacy policies.

Admissions uses AI to read hospital documentation. Recommendations should be reviewed by your clinical and MDS team before entry. Admissions supports, and does not replace, clinical and coding judgment. Screenshots in this article show masked demo data.
